About White Mountain Medical Center Family Practice

We care for patients of all ages. We diagnose and treat patients with chronic conditions such as diabetes, hypertension and asthma. In addition, we treat acute care needs such as minor cuts, sore throats and headaches. All of our patients have the added convenience of same day appointments. Our services include:

Requesting Medical Records

Medical records are available upon request. For your request to be processed, you will need to fill out the Authorization to Use & Disclose Health Information form, which is available for download on the Medical Records page, as well as complete instructions on how to request your personal medical records.

Requesting Prescription Refills

If you need a routine prescription refill, please contact your pharmacy directly and request a refill. Your pharmacy will contact our office on your behalf — no need to contact our office directly.

If You Have A Serious Illness Or Injury

Call 911 or go to your nearest emergency room for serious illness or injuries, including:

  • Chest pain (suspected heart attack or stroke)
  • Amputation of limbs or deep wounds with severe bleeding
  • Spinal injury
  • Sudden confusion, weakness, dizziness, numbness, severe headache or difficulty speaking or breathing
  • Sudden change in vision
  • Head injuries with loss of consciousness or new seizures
  • Burns involving face or large areas of the body
  • Vomiting blood
  • Smoke or chemical inhalation
  • Poisoning, overdoses or suicidal behavior
  • Physical or sexual assault
  • Pregnancy complications including labor, bleeding, fluid leakage, decreased fetal movement
