At Rochester Primary Care, our team offers patients in Rochester and the surrounding communities care for the whole family, from newborns to the elderly. As the result of merging Rochester Hill Family Practice and Skyhaven Internal Medicine, Rochester Primary Care creates access for patients to a larger team of experts who work together to provide continuity of care from pediatrics to adulthood and through the geriatric years.

Our providers treat acute and chronic illnesses by providing preventive care and health education for men, women and children of all ages. Because family history and hereditary factors provide significant information in determining a patient's health status, having a single physician treat all members of a family can be beneficial. We offer complete healthcare services by appointment for the entire family.

Pediatric care

Our providers care for pediatric patients, from newborns to adolescents. They provide services such as:

• Access to your provider with Telehealth
• Childhood immunizations/flu shots for the family/vaccinations
• Developmental screening
• Management of chronic conditions like ADHD, asthma, and diabetes
• Newborn care

• Same-day sick care visits are often possible

Adult primary care

Our practice focuses on preventive health care and provides adults, from young adulthood all the way through mature adulthood, the care they need to have their best quality of life. Services we offer adults include:

• Annual physicals and Medicare Annual Wellness Visits
• Coordination of at-home or nursing home care
• Management of chronic conditions like asthma, diabetes, thyroid disorders, and hypertension
• Medication management
• Preventive healthcare counseling
• Same-day acute care visits are often possible
• Scheduling, reviewing, and interpreting diagnostic testing

With our Patient Portal, you will have access to test results and can communicate with our team. We also provide access to your provider through Telehealth.

Requesting prescription refills

We have dedicated pharmacy technicians to assist you with your prescription requests, pre-authorizations, and refills. We will work with your pharmacy on your behalf to help support and maintain your care.
